DOH PROVIDES COMMUNITY WITH VACCINE RECORD INFORMATION
The Department of Health (DOH) is providing the community with information on how to obtain copies of your COVID-19 receipt/record. With vaccine passports being required in Ontario and Quebec in the very near future, the DOH would like to ensure the community is aware of the process to obtain their vaccine record.
Beginning on September 22, 2021, proof of COVID-19 vaccination will be required to access many indoor settings in Ontario. On October 22, 2021, Ontario will be moving towards an upgraded proof of vaccination, which includes a QR code, followed by an app consumers and businesses can download and use. Quebec began requiring proof of vaccination on September 1, 2021 to access many indoor public venues.
Community members who received their COVID-19 vaccine through the Community Health Program clinics can obtain a copy of your vaccine receipts via the following website: https://covid19.ontariohealth.ca. You will need your green Ontario health card number, date of birth, and postal code. If you do not have a photo health card, call the Provincial Vaccine Booking Line at 1-833-943-3900 (8 a.m. – 8 p.m., 7 days a week).
Any community members who have received their COVID-19 vaccines from outside of Ontario (Saint Regis Mohawk Tribe clinics, or elsewhere) can report their immunization to the Eastern Ontario Health Unit’s COVaxON Vaccination Management System by visiting www.eohu.ca, and clicking on the link labeled “Register a Record of COVID-19 Vaccination Received Outside of Ontario” at the bottom of the page. (For QHIP card holders, enter all 0’s into the card information to bypass this section). Upload as much information as possible on the registration page (first name, last name, date of birth, phone number, email, postal code for which district you reside). Upload a picture of your CDC vaccination card to the site (ensure the card has BOTH doses and LOT number showing). It takes approximately 72 hours to be register into the COVax system once everything has been submitted. If you don’t have internet access, you can call 1-866-318-0522 for assistance.
Community members who received their vaccine in Quebec and are in need of their Proof of Vaccination Receipts can access the “Self Service Portal” on the website www.quebec.ca. Once at the website, click on the “Health” link, followed by the “Health Issues link,” then the “A to Z” link. Scroll down to “Coronavirus” and click on “Vaccination Appointments.” Scroll down the web page to “Accessing the Self-Service Portal” link. Fill out the required information on the page to receive a digital copy of your vaccine receipt.
Community members who received their vaccination outside of Quebec, and would like to register their vaccine record with Quebec, can go to the Clic Santé Portal at www.clicsante.ca to register their vaccine record. To register your vaccine record, click on “Choose a service”, and from the dropdown menu, select “COVID-19 Vaccine – Troubleshooting,” enter in your postal code, and select from the menu the closest health service. You will be required to book an appointment with the nearest service provider to have your vaccine record registered with Quebec.
Ontario and Quebec will accept proof of vaccinations received outside the provinces if a printed proof of vaccination is presented with accompanying identification. Ontario will also accept an electronic receipt.
