EMAIL COMMUNITY HEALTH TO RECEIVE YOUR OFFICIAL ONTARIO COVID-19 VACCINE RECEIPTS
The Community Health Program is notifying the community that the program has established an email address for community members who have received their COVID-19 vaccination through Community Health, and are requesting their official Ontario COVID-19 vaccination receipts. Email requests can be sent to covidvaccines@akwesasne.ca.
In your email request, please include your Name and Date of Birth. The Community Health Program staff will input the record of your dose into the Ontario Health Provincial System. Once your information has been input into the system, the province will send the receipt via email with your official Ontario COVID-19 vaccination receipts in 1-3 business days.
