DEPARTMENT OF HEALTH WELCOMES NEW HOME CARE/HOME SUPPORT PROGRAM MANAGER

The Mohawk Council of Akwesasne’s Department of Health would like to extend a warm welcome to Chelsy Plante. Chelsy joins the Department of Health as the Home Care/Home Support Program Manager. 


Chelsy is Anishinaabe (Ojibwe) from Nagagamisis in Northwestern Ontario. She has her Bachelor of Sciences in Nursing (BScN) from Lakehead University and graduated with a special certificate in Palliative Care. She spent a majority of her RN career in Home & Community Care, while developing Preventive Health programs and Chronic Disease management. She holds our Elders to a high regard and is passionate about ensuring the highest quality of care during older age and end-of-life.


Chelsy also carries a deep passion and understanding of her cultural roots. She advocates for equality within the western healthcare system while supporting First Nation Traditional Health for her patients. She teaches First Nation history to first year nursing students to ensure a thorough understanding of wholistic health for the frontline workers in training. Chelsy is dedicated to her work as a leader in health care for First Nation people and she is honored to be in her new position as the Program Manager for Home Care/Home Support.
