DEPARTMENT OF HEALTH ANNOUNCES NEW COMMUNITY PARAMEDIC INDIGENOUS PATIENT NAVIGATOR PILOT PROGRAM
The MCA Department of Health would like to notify the community about the new Community Paramedic Indigenous Patient Navigator. Daryl Diabo will fill the role of this new program.
Daryl brings over 35 years of experience as a paramedic, having served for many years with the Akwesasne Mohawk Ambulance Service.
The Community Paramedic Indigenous Patient Navigator (CPIPN) program is a pilot program that aims to bridge some of the gaps in Primary Care by bringing forth some primary care services into the home setting. The goal of this program is to reduce patient exacerbations and crisis which would require paramedic response. The program also aims to reduce emergency room visits and hospital admissions.
Through this new program, the CPIPN will collaborate with potential client’s family doctors, which will reduce the need for clinic visits while assisting community members with navigating Quebec Health services and healthcare system and advocating for their healthcare needs.
The CPIPN will work with clients by providing in-home chronic disease management, including preventative care and education. The CPIPN will be working collaboratively within the Clinical Care Services sector.
This new pilot program is expected to launch on Wednesday, April 2, 2025. Community members interested in becoming clients of the Community Paramedic Indigenous Patient Navigator can self-refer or be referred to the program by their healthcare providers or a nurse.
