HCHS PROGRAM OFFICES NOW LOCATED IN ALL THREE NORTHERN DISTRICTS OF AKWESASNE

The Mohawk Council of Akwesasne’s Home Care/Home Support (HCHS) Program is providing the community with an update on their transition period. HCHS has now completed their transition from offices on Kawehno:ke to occupying office spaces in all three districts of Akwesasne. This transition was enacted to maximize the services accessible to their clients and to the Akwesasne community.

 

HCHS has offices located at Iakhihsohtha Lodge, Kanonhkwa’tsheri:io Health Facility, and the Tsiionkwanonhso:te Long-Term Care Facility. HCHS will be resuming its regular program operations and services, such as home visits, Home Medical Equipment deliveries, referral services, transportation services, etc.
